Senior Full-Stack Healthcare Software Developer EHR EMR PACS & AI

About the role
We are looking for an experienced Full-Stack Developer with strong hands-on experience in healthcare software development , including EHR/EMR systems, PACS/DICOM imaging, medical image post-processing, clinical charting, and AI integration . The ideal candidate should understand both healthcare workflows and modern software architecture and be comfortable working across frontend, backend, databases, APIs, cloud infrastructure, and medical imaging systems.
Key Responsibilities
- Develop and maintain full-stack healthcare applications
- Build and improve EHR/EMR modules
- Develop clinical patient charting and documentation workflows
- Build modules for: Patient demographics Appointments and scheduling Encounters SOAP/progress notes Provider documentation Orders Medications Diagnoses Lab and imaging results Billing/RCM workflows Integrate healthcare systems using HL7, FHIR, APIs, and web services
- Work with PACS and DICOM imaging systems
- Build DICOM upload, transfer, viewing, routing, and study management workflows Experience with DICOMweb, C-STORE, C-FIND, C-MOVE, STOW-RS, WADO-RS , or similar technologies Integrate imaging viewers and post-processing tools
- Develop or integrate medical image post-processing pipelines
- Work with CT/Cardiac imaging workflows where applicable Integrate AI/ML models into clinical and imaging workflows
- Create AI-assisted clinical charting and provider-review workflows Integrate LLMs such as OpenAI or other healthcare AI models
- Build AI-assisted: Chart summaries Clinical documentation Provider suggestions Medical coding assistance Imaging analysis workflows Report generation
- Ensure AI output remains reviewable and controlled by healthcare professionals
- Build secure APIs and scalable backend services
- Work with cloud infrastructure such as AWS Implement secure healthcare data storage and access controls
- Maintain audit logs, role-based access, and healthcare security standards
- Troubleshoot frontend, backend, database, PACS, and integration issues Preferred Technical Experience Strong experience with technologies such as: Frontend React TypeScript JavaScript Next.js or Vite Modern UI frameworks Backend Node.js Express Python REST APIs WebSockets / Socket.IO Databases PostgreSQL MongoDB Supabase or similar platforms Healthcare EHR / EMR development HL7 FHIR DICOM DICOMweb PACS Orthanc or similar PACS systems Clinical documentation/charting workflows Medical Imaging / Post-Processing CT / CTA / CCTA imaging workflows Medical image segmentation 3D reconstruction Vessel analysis MPR / MIP / curved planar reconstruction Image post-processing pipelines MONAI, TotalSegmentator, VMTK, SimpleITK, ITK, or similar tools would be a plus AI Integration OpenAI API or similar LLM platforms AI-assisted clinical documentation Medical image AI integration Model APIs Python ML pipelines Prompting and structured AI output Human-in-the-loop/provider-review workflows Cloud / DevOps AWS EC2 S3 Docker Linux VPS deployments Git/GitHub CI/CD Required Experience Strong full-stack development experience Previous experience working with healthcare applications Hands-on EHR/EMR development experience Understanding of clinical workflows and patient charting Experience with APIs and third-party healthcare integrations Experience with PACS/DICOM or medical imaging systems is highly preferred Good understanding of healthcare data privacy and security Ability to independently troubleshoot complex systems Highly Preferred Candidates with experience in one or more of the following will be strongly preferred: Cardiology software Radiology software CCTA/Cardiac CT PACS integrations Medical image post-processing AI-assisted clinical documentation AI medical imaging Revenue Cycle Management / medical billing systems Building healthcare platforms from the ground up
